Coaches and creators

Tell me about you and your work

About 10 minutes, mostly tap to pick. Skip anything you're not sure about.

Only your name and one way to reach you are needed. Anything you skip, I'll fill in facts like hours and prices from your social pages, or ask you once. I only use a photo that shows a child if a parent has said it's OK.

This form is about you, the coach or creator. Please don't enter any player's or student's name or details. You need to be 18 or older to send this form.

Here for a business or shop? Use the business form. Just looking? Get a free sample first, with no obligation.

After you send this, I reply within 48 hours. Next comes your one-page agreement (the terms, in plain words); you sign it online through Square’s “Review & sign” email (or reply “I agree”). Nothing is billed until you do.

Part 9 of 16

Sign-up or waiver form

Do you have a parent sign-up or waiver form?

Your site only mentions it if you want. I don't write waivers.

Part 10 of 16

Your story

Optional. Write it the way you'd say it. I'll tidy it up and show you before anything goes live.

Where you played, coached, trained or studied, and why you do this.

Only ones you really hold and can show me, like a coaching license or a CPR card.

Part 11 of 16

Kind words

Optional.

Word for word, please, only from people who said you can share them. Add their first name, or no name. Please leave out any child's name.

Part 12 of 16

Photos

Optional, up to 3. One file per box.

Files can add up to 8 MB in total each time you send this form. A phone photo is often 2 to 5 MB, so attach one or two here, then text or email the rest to me. If you send photos that way, tell me if a child is in one and that a parent said it's OK. On a slow connection, send fewer files.

You coaching, your gym or your setup.

Photos with children in them

Not sure about a photo? Leave the kids out of it, or I'll blur them or skip that photo.

Part 13 of 16

All prices are plus 6% Kentucky sales tax, shown on your invoice. Half to start and half before your site goes live (or all at once, if you prefer; if you pay it all and the job ends before launch, the second half comes back), by Square invoice, after you sign the one-page agreement; your deposit is fully refundable until I deliver your first draft. One round of changes is included, and the site is yours once the build price is paid and no longer refundable. The terms, in plain words.

Which package?
Add a care plan?
Do you have a domain?

Your domain is your web address, like yourname.com. It's in your name and costs about $10 to $20 a year.

The company you pay for it each year.

When do you need it?

Part 16 of 16

Anything else

Camps coming up, things people always ask you, anything you'd like on your site.

I use your answers only to build your website. I don't sell them. How I handle what you send.
